Resident Enrollment Requirements

4. Proof of Residency in RIVER ROUGE (3 pieces: 1 must be a current Driver's License or State ID)

8. I.E.P. for Special Education
9. Transcripts (9th-12th grade)/Report Card (K-8th grade)

Non-Resident, Schools of Choice Enrollment Requirements

1. Birth Certificate
2. Immunization Records
3. Physical
4. Proof of Residency in WAYNE COUNTY (3 pieces: 1 must be a current Driver's License or State ID)
5. Schools of Choice Application
6. Suspension/Expulsion Policy
7. Discipline and Attendance Letter or Report
8. District Enrollment Form
9. Emergency Card
10. Lunch Form
11. I.E.P. for Special Education
12. Transcripts (9th-12th grade)/Report Card (K-8th grade)

Immunization Requirements

• DTP - 4 doses - Last dose no later than 10 years ago. (4th dose must be on/after 4th birthday)
• Polio - 3 doses (3rd dose must be on/after 4th birthday)
• MMR (Measles, Mumps, Rubella) - 2 doses
• Hepatitis B Series - (3 shots needed but series must be started)
• Varicella - 1 dose (or must have had Chicken Pox)
  
      2 doses (if initiated after 13th birthday)
